Part-time benefited; Minimum 64 hrs/pp; Varied shifts to include 8a-4:30p, 6a-6p, 6a-4:30p, on-call rotation required. Minimum of 3 years Acute Care nursing experience required.
JOB SUMMARY
The Acute Care Coordinator will serve as a direct extension of the Acute Care Director in Leadership and the Coordination of Staff assignments, scheduling and staff education. Responsibilities would include monitoring of direct patient on the Acute Care Unit ranging in age from pediatric to geriatric, who may present at any point on the continuum from illness to wellness. Performs all aspects of direct patient care and works with AC Staff, Providers and other disciplines/ancillary services in the care they provide. May serve as an alternative attendee in committees/projects the AC Director serves on or chairs and serves as the staff contact for issues when the AC Director is not available.
MAJOR JOB FUNCTION

Monitors staffing assignments and day to day staffing needs.

Promotes and contributes to a positive, problem-solving environment.

Assists the department director in monitoring performance standards, providing competency testing, providing in-services and education, holding and documenting meetings following standardized meeting format, conducting new hire orientation, assessing performance annually, reviewing and computing hours on time cards for payroll use and maintaining ongoing list of departmental goals.

Participates in drafting and revising policies and procedures relating to the Acute Care and services provided.

Participates in specific patient quality assessment activities, including chart review and follow up phone calls.

Works with Acute Care Director in preparing and monitoring the budget for the operation of the department.

Maintains established hospital and departmental policies and guidelines, objectives, quality improvement programs, safety, environmental and infection control standards.

Enhances professional growth and development of self and others through participation in educational programs, current literature, in-service meetings and workshops.

Complies with certification/recertification requirements of the position and attends continuing education courses to remain current in the field.

Provides direction and leadership to staff, students and new graduates in conjunction with the Acute Care Director.

Maintains a professional attitude and observes proper ethics.

Plans, implements and evaluates all levels of patient care; performs age appropriate assessments and provides safe and effective care across the lifespan to ensure that practice aligns with most up to date evidence based research.

Documents, using medical terminology, and maintains an Acute Care record that is medically and legally acceptable, reflective of all nursing care provided and the patient's response to the care given.

Collaborates with all providers, peers, ancillary department and outside agencies to provide efficient, cost effective, optimum patient care.

Coordinates the care needs of multiple patients. Able to prioritize patient care to ensure patient needs are met in a timely fashion, and that quality care is delivered and the patient flow is enhanced.

Verbally communicates patient needs in a concise, reasoned fashion. Is able to advocate for the patients under her care through the use of communication with providers, peers and ancillary departments. This may include using the chain of command when needed.

Assesses patient and family knowledge and discharge needs and provides for same.

Monitors life support and automated technical equipment, performs routine and complex treatments including medication administration and emergency resuscitation protocols. Inserts and safely administers IV fluids, blood and blood products, and other parenteral fluids.

Responds to crisis in a calm, efficient manner, directing others concisely. Uses delegation appropriately. Able to rapidly solve and deliver calm, efficient care in a high stress situation.

Complies with legal consent, transfer protocols, and other legislative requirements that preserve patient rights and ensure appropriateness of care.

Performs nursing procedures safely and efficiently.

Participates in fire, safety, disaster and infection control educational programs and exercises.

Performs related work as required.

EDUCATIONAL REQUIREMENTS
-
Graduation from an accredited school of nursing, BSN required within three years of hire.
-
Current licensure to permit to practice as a Registered Nurse in the state of Wisconsin.

Current certification or eligibility to become BLS, ACLS and PEARS certified.
-
Minimum 3 years of Acute Care nursing experience required.

Acute Care certification required within 2 years of hire.

JOB REQUIREMENTS

Regularly required to use hands to finger, handle or feel objects, and talk and hear.

Frequently required to stand, walk and reach with hands and arms. The ER Director is occasionally required to sit, climb or balance, stoop, kneel or crouch, and detect odors.

Must occasionally lift and/or move up to 50 pounds.

Good hearing is necessary to receive detailed information through oral communication and to make fine discriminations in sound.

Visual acuity is needed to assess color changes, to verify accuracy of written materials.

Computer/keyboard skills required. Must have the ability to operate in a Windows environment.
WORKING CONDITIONS
Must understand and accept the possibility of exposure to inside environmental conditions, such as infectious/communicable diseases, noise, blood and bloodborne diseases, chemicals and/or chemical fumes, odors, gases, dusts, and physical injury/verbal abuse from an out-of-control patient. Frequent exposure to distressed patients, families or visitors. Must be able to function effectively under stressful situations.
