Case Manager

The region’s leading network of healthcare providers, Inspira Health is seeking a full-time Case Manager to join us at our Vineland, NJ location. You will play a critical role in facilitating the patient’s hospitalization, from pre-admission through discharge. Responsibilities include:

• Collaborating with physicians, nurses, social workers and other healthcare professionals to ensure the highest quality of patient care
• Performing initial and ongoing assessments to ensure effective transitions and prevent hospital readmission
• Ensuring the discharge plan is consistent with the patient’s clinical course, continuing care needs and covered services
• Providing referrals to social work services to patients and/or families that require further assistance
  
  

Qualifications:

• BSN required
• Licensed Registered Nurse in New Jersey required; Case Management Certification preferred
• 3+ years of experience in a hospital setting; 1+ years of case management experience preferred
• Experience in assigned unit or service line with previous utilization management
• Previous experience with regulatory and compliance requirements preferred
• Strong communication, interpersonal and administrative skills
