We're seeking a Behavioral Health Care Manager to join our team in delivering COCM services. You'll work directly with patients, primary care providers, and psychiatric consultants to provide care coordination, brief interventions, and systematic outcome tracking. This is a mid-level position ideal for someone with behavioral healthcare experience who is advancing their expertise in integrated care.

Requirements

Required:

• Bachelor's degree in behavioral health, social work, counseling, psychology, or related field
• Experience providing direct behavioral health services
• Strong care coordination and communication skills
• Ability to work collaboratively with interdisciplinary teams

Preferred:

• Experience in primary care or integrated care settings
• Familiarity with measurement-based care and patient registries
• Currently pursuing Master's degree in behavioral health field
• Knowledge of evidence-based brief interventions
