Description
Methodist Hospital, part of SA Health, is seeking an experienced Case Manager RN to join our Healthcare & Medical Services team. In this role, you will coordinate patient care plans, conduct assessments, facilitate discharge planning, and collaborate with interdisciplinary teams to ensure optimal patient outcomes and efficient use of resources. You will advocate for patients and families, connect them with community resources, and ensure compliance with regulatory standards. Complete comprehensive clinical and psychosocial assessments to identify patient and family needs, risks, and discharge barriers
Develop, implement, and regularly update individualized care and discharge plans in collaboration with the interdisciplinary team
Facilitate timely and appropriate use of hospital and community resources, including post-acute services and referrals
Perform utilization review activities, ensuring appropriate level of care and compliance with payer and regulatory requirements
Monitor length of stay and work with providers to support efficient, quality-driven care and transitions
Educate patients and families on care plans, disease processes, medications, and available support services
Advocate for patients and families to promote safety, quality outcomes, and satisfaction
Maintain thorough, accurate, and timely documentation of case management activities in the electronic medical record
Participate in quality improvement initiatives and attend required meetings, trainings, and huddles as needed in a PRN capacity
Clinical nursing assessment and critical thinking
Hospital case management and discharge planning
Utilization review and level-of-care determination
Knowledge of payer guidelines and regulatory standards
Care coordination and transitional care planning
Patient and family education
Interdisciplinary communication and collaboration
Time management and prioritization in a fast-paced setting
Electronic medical record (EMR) documentation
Knowledge of community and post-acute care resources
Conflict resolution and patient advocacy
Data interpretation related to length of stay and utilization

Current Registered Nurse (RN) license in the state of Texas
Basic Life Support (BLS) certification
Advanced Cardiovascular Life Support (ACLS) preferred or obtained within specified timeframe
