Perform comprehensive assessments of patients’ clinical, psychosocial, and discharge planning needs
Develop, implement, and regularly update individualized care and discharge plans
Coordinate services and resources across the continuum of care, including post-acute and community services
Collaborate with physicians, nurses, social workers, therapists, and other team members to support optimal patient outcomes
Conduct utilization review and work with payers to support appropriate level of care and authorizations
Advocate for patients and families, helping to remove barriers to care and access to services
Educate patients and caregivers on diagnoses, treatment plans, medications, and community resources
Facilitate safe and timely transitions of care, including discharge planning and follow-up arrangements
Document assessments, plans, interventions, and outcomes accurately in the electronic medical record
Participate in interdisciplinary rounds and quality improvement initiatives to enhance care coordination and patient outcomes
