We are seeking an experienced Case Manager RN for an acute care facility in Mission Hills, CA.

Assignment Details:

• Start Date: ASAP
• No Floating Required
• Patient Ratio: 1:20
• Weekend Rotation Required based on department needs

Requirements:

• Active CA RN License required (pending license accepted but must be active by start date)
• Minimum 2 years of acute care hospital Case Manager experience
• Strong experience in Care Coordination and Discharge Planning
• Acute care hospital experience is required
• First-time travelers may be considered and will be clinically screened

Additional Information:

• Maximum RTO: 7 days
• Acute Care Facility setting

This position is ideal for a Case Manager RN with recent hospital-based case management experience who is skilled in coordinating patient care, discharge planning, and interdisciplinary collaboration.
