DescriptionAs a Case Manager RN, you will coordinate patient care, ensuring that discharge planning is efficient and meets the needs of patients. Your role includes working closely with multidisciplinary teams to manage patient flow and care quality. This position is pivotal in navigating the complexities of patient cases, particularly in pediatrics, and offers the ability to make a significant impact on patient outcomes.Highlights
- Pediatric experience required with a focus on acute care
- Opportunity to work in a Magnet facility
- Epic EMR experience preferred
- 5x8 day shifts with weekends required every other week
- Located in scenic Anchorage, Alaska
- BLS preferred
- ACM Certification Preferred
- AK RN License
- Epic (EMR) experience required
- Minimum 2 years of experience in case management
